Add an upgrade for the SQL metadata schema.

Databases::create() reuses an existing _metadata table without adding new columns. A scoped permission stores its grants in columnPermissions. SQL adapters include that attribute in their INSERT or UPDATE statements. On an installation created before this field existed, creating or updating a collection with a scoped grant can therefore fail because the _metadata table has no columnPermissions column.

Add an adapter-aware metadata schema migration for existing SQL tables. Keep Mongo, Redis, and Memory on their current paths because they do not require a physical column.

Review comment at @src/Database/Database.php around lines 2571 - 2577:
Add an adapter-aware `_metadata` schema upgrade so existing SQL tables gain the
`COLLECTION_COLUMN_PERMISSIONS` column before SQL inserts or updates use it.
Keep Mongo, Redis, and Memory on their existing paths; anchor the change around
`Databases::create()` and the metadata attributes shown in the diff.
